If you would like a representative to deal with any medical matters relating to you, including the collection of repeat prescriptions, you must provide us with a written authority with their name, relationship to you and signed by yourself. A telephone number for them would also be beneficial.
If written authority is not recorded within your medical records, your representative will NOT be given any information about you.
